Cahokia Creek rises in southern Macoupin County and passes southwesterly through Madison County. Its original course wound through Cahokia, the site of a pre-Columbian Native American metropolis, before emptying into the Mississippi River opposite St. Louis. The creek's lower end has since been diverted by canals.
