Johnstone received an education in Scotland before immigrating to the United States around 1818. After spending two years in Ohio, he returned to Scotland to study medicine. While in Scotland, he married Jane Carlyle, and the couple had three children. After Johnstone's studies were complete, he returned to Ohio to practice medicine near Chillicothe. In 1840, he moved west, settling permanently in Bloomington, Illinois. After moving to Illinois, he practiced little medicine, instead he spent his time farming and in civil engineering.
